Who is Forme designed for?
Women navigating midlife who want to better understand their changing bodies and build a connected strategy for body composition, strength, nutrition and wellbeing. The consult helps you explore whether this care model fits your priorities; clinical eligibility is assessed separately.
Do I need to want weight loss or medication?
No. Weight Loss is one of three pathways. Performance and Longevity focus on other priorities, and medication is not a requirement or a guaranteed part of care. Clinical options are considered individually.
How individualized is the plan?
Your goals, history, comprehensive labs, current strength and everyday habits inform your starting plan. Clinical assessment guides medical decisions; coaching helps tailor nutrition, movement and implementation. Your response helps guide adjustments.
How do clinical care and coaching work together?
Dr. Christine Schaffner, ND, leads clinical assessment, interpretation of labs and prescribing when appropriate. Sami leads strength, nutrition and behavior coaching. Coaching supports implementation and accountability; medical questions and treatment decisions stay with your clinician.
Where is Forme available?
The regions listed in our current membership preview are Washington, Idaho, Utah, Oregon, California, Arizona, Montana and Washington, D.C.
Availability is subject to confirmation of current licensure, scope and service eligibility where you are physically located for care. Please confirm your location during the consult before enrolling.
What happens after the first 90 days?
The initial commitment establishes your foundation. Forme then evolves into an ongoing monthly membership, with support shaped around your progress and priorities. Ongoing pricing, inclusions, renewal and cancellation terms are still awaiting final confirmation in this preview and should be reviewed before enrollment.
What happens on the free 15-minute consult?
We talk about what you want support with, explain the Forme approach and discuss fit, availability, costs and next steps. It’s a conversation, with no pressure to join. It does not provide a diagnosis or prescription.
